Diabetes & Chronic Condition Support in Nairobi & Kiambu
Day-to-day help for parents living with diabetes, high blood pressure or heart conditions: meals, routine, reminders and watching for warning signs.
Diabetes, high blood pressure and heart conditions are common among older Kenyans. Managing them well depends on daily habits — regular meals, taking medicine reliably, staying active, and noticing problems early.
How I help
- Meals: preparing meals that follow the diet the doctor or nutritionist recommended — portion sizes, less salt or sugar, regular mealtimes
- Medication reminders: for tablets the doctor has prescribed, with a simple log for family
- Routine and activity: gentle walks and movement, as their doctor advises
- Readings: if the client or a nurse has been trained to use a home glucose or blood pressure monitor, I can remind them, help them record readings and share them with the family
- Clinic visits: accompanying them to their regular diabetes or blood pressure clinic
Watching for warning signs
For diabetes, low blood sugar can cause shakiness, sweating, confusion or unusual behaviour. Very high blood sugar can cause extreme thirst, frequent urination and drowsiness. I follow the plan agreed with their doctor, contact the family, and call for emergency help if they become very drowsy, confused or unresponsive.
I also watch for foot problems in people with diabetes — any sores, blisters or colour changes are reported to the family promptly so a nurse or doctor can check them.
